What Is a Patient Navigator?

A Patient Navigator is a specialized member of your care team whose primary focus is helping you successfully navigate every aspect of your cancer care journey.

Depending on your needs, your Patient Navigator may bring a nursing background, care coordination expertise, or experience connecting patients with community and support resources. While their backgrounds may vary, their mission remains the same: To help remove barriers to care and ensure you receive the support you need when you need it.

Patient Navigators work closely with physicians, nurses, caregivers, and other specialists to help coordinate care, answer questions, and connect you with valuable resources throughout treatment.

Araceli Anaya

Oncology Patient Navigator

My name is Araceli Anaya, I proudly worked in the education sector for over 15 years, beginning my career as a preschool teacher and later serving as a Parent Coordinator and Home School Liaison within the Migrant Education Program for Kerman Unified School District as well as Fresno County Superintendent of Schools. Through these roles, I developed a deep passion for supporting families, building strong community connections, and helping individuals access the resources and opportunities they need to succeed.

After many rewarding years in education, I decided to pursue a career change and transitioned into community health, where I worked as a Community Health Navigator specializing in telehealth and case management. In this role, I focused on engaging and recruiting members from underserved communities and connecting them with Enhanced Care Management (ECM) services and other essential health resources.

What I love most about being a Patient Navigator at California Cancer Associates Reach & Excellence is the opportunity to make a meaningful difference in someone’s life. I enjoy helping members understand and navigate their healthcare benefits, connecting them with the appropriate services, and making sure they receive the care and support they need. I value the relationships I build with the members I serve and the trust they place in me throughout their healthcare journey.

Outside of work, I love spending quality time with my family and friends and my two huskies, traveling, painting, and exploring my creative side. I also have a special love for owls and enjoy collecting all things owl related.

Terri Brisco

Oncology Patient Navigator

Terri Brisco is a dedicated Patient Navigator with a passion for helping individuals access the healthcare resources, services, and support they need. Committed to patient advocacy, she works closely with patients, families, and healthcare teams to coordinate care, overcome barriers, and ensure a positive healthcare experience.

Outside of work, Terri enjoys spending time with family and friends, exploring the mountains, and giving back to her community. She values meaningful connections and strives to make a positive impact in the lives of those around her.

What Terri loves most about being a Patient Navigator is the opportunity to support patients during some of the most challenging times in their lives. She finds fulfillment in guiding individuals through complex healthcare systems, empowering them with knowledge and resources, and helping them feel heard, supported, and cared for every step of the way.

Gina Pereida

Oncology Patient Navigator

My name is Virginia Pereida, though most people know me as Gina. I have proudly worked at cCARE for 17 years, dedicating my career to supporting patients and providing exceptional care.

I began my journey at cCARE as an Administrative Medical Assistant, where I served patients and physicians for many years. Following Dr. Jawien’s retirement in 2026, I pursued my certification as an Oncology Patient Navigator. After earning my certification, I transitioned into my current role as a Patient Navigator.

Working directly with patients is one of my greatest passions. I am committed to helping patients navigate their cancer journey, providing guidance, support, and resources to ensure their needs are met throughout their treatment experience. Being there for patients and their families during challenging times is one of the most rewarding aspects of my career.

Outside of work, I enjoy watching San Francisco 49ers football, camping, taking long drives, and spending quality time with my family.

Kemonie Ray, LVN

Oncology Patient Navigator

Kemonie Ray is a Licensed Vocational Nurse with more than four years of experience in patient care, oncology navigation, care coordination, and healthcare operations. She is passionate about supporting patients throughout their healthcare journey, helping them access resources, understand treatment plans, and navigate complex care needs. Kemonie brings expertise in patient education, utilization support, EMR documentation, and interdisciplinary collaboration. 

Lenzy Rubi

Oncology Patient Navigator

Lenzy Rubi has over 12 years of experience in oncology and a background in social work, providing compassionate, patient-centered support to individuals facing cancer. She partners closely with patients, caregivers, physicians, and multidisciplinary teams to coordinate care, provide education, connect patients with financial assistance and community resources, and help remove barriers to treatment ensuring every patient feels informed, supported, and empowered.
