Notifications are being provided regarding a recent incident experienced by our third-party practice management software provider, Unlimited Technology Systems (“ULS”).

What happened

ULS learned that an unauthorized third party gained access to and obtained data that was maintained by ULS. ULS determined through an investigation that, at least as early as October 5, 2025, an unauthorized third party gained access to information maintained on ULS datacenters. ULS notified us of this incident and, on May 20, 2026, provided us with a list of patients whose information may have been involved in this incident, following a complex data review and analysis.

Note – this incident did not involve our computer systems and did not disrupt patient care.

What information was involved

The information potentially involved varied by patient but may have included patients’ names and other information included within the practice management software, such as health insurance and patient balance information (insurance policy numbers or claims/benefits information), medical information (medical record number, dates of service, diagnosis information), driver’s license numbers, insurance cards, intake forms, and other personal information (date of birth, email, address, phone number, or demographic information). For some patients, the incident may have also involved their Social Security numbers.

What we are doing and what can you do

ULS is offering all notified individuals complimentary credit monitoring and identity protection services. ULS has also established a dedicated, toll-free incident response line to answer questions that individuals may have about the incident. If you believe you were affected, but did not receive a letter concerning this incident, please contact the incident response line at the number below.
