LaSalle Medical Group Update for cCARE Physicians, Staff, and Patients

Effective April 1, 2025 cCARE will no longer be contracted with LaSalle Medical Group.

Why Is This Happening?

Despite our best efforts to work in good faith with LaSalle Medical Group, we were unable to reach an agreement. As a result, cCARE will no longer be a contracted provider with LaSalle Medical Group after April 1, 2025. However, we remain committed to supporting you through this transition.

When Will My Insurance Be Out Of Network?

Effective April 1, 2025, cCARE will no longer be contracted with LaSalle Medical Group. 

Are We Still Accepting New LaSalle Medical Group Patients Before The Termination Date?

Yes, we will accept new LaSalle patients until March 31, 2025. 

Continuation of Care: Keeping You Connected to Your Provider

If you are currently receiving treatment at cCARE and are affected by this change, you may be eligible to continue your care with us for up to one year through the Continuation of Care policy.

What Is Continuation Of Care?

Continuation of Care is a program that may allow you to temporarily continue treatment with us even after we are out of network. It’s specifically designed to support patients in the middle of active treatment.

Am I Eligible For Continuation Of Care?

Eligibility is determined by LaSalle Medical Group, but typically, patients receiving ongoing treatments like chemotherapy or post-surgical care are eligible. We recommend reaching out to LaSalle’s Member Services to start the process.

What Happens If I’m Not Eligible For Continuation Of Care?

If Continuation of Care is not an option, we will work closely with you to explore other paths for care, such as transitioning to an in-network provider or discussing alternate payment options if you wish to remain with us. 

Can I Pay Out Of Pocket? What Is The Cost?

Yes, if you wish to continue receiving care at cCARE after March 31, 2025, you have the option to self-pay for services. We can provide information on pricing and payment options to help you make the best decision for your care. 

Will I Be Able To Continue To See My cCARE Doctor After March 31, 2025?

Your ability to continue seeing your provider at cCARE after March 31, 2025, will depend on several factors. If you qualify for Continuation of Care, you may continue receiving treatment with us for up to one year. If not, you will need to contact your insurance provider for options.  If you are interested in continuing your care with our physicians, you would need to explore alternative insurance options or pay as a self-pay patient with us. 

Will I Have To Change Doctors Or Clinics?

If Continuation of Care is approved, you may not have to change doctors right away. However, after the Continuation of Care period ends, you may need to transition to a provider in LaSalle’s network unless you choose to alternative options allowing you to continue your care with us. 

What About My Upcoming Appointments Or Treatments?

All appointments and treatments will proceed as planned through March 31, 2025. After April 1, we will work with you to ensure a seamless transition based on your Continuation of Care status or consideration of alternative personal insurance coverage options. 

Can I Change My Insurance To Continue To See My cCARE Doctor?

You may have the option to switch to an insurance plan that includes cCARE as an in-network provider. We recommend reaching out to your employer’s benefits department or an insurance broker to explore your options. We can provide you with information on brokers if you do not have a broker. 

What Other Insurances Does cCARE Accept?

cCARE accepts Medicare, Medical, and most insurances.  Please ask our dedicated care team for details.
